Understanding Your Patient Responsibility
Your financial responsibility is determined by your insurance plan. We follow your insurance plan's decisions regarding deductibles, copays, and coinsurance. We do our best to estimate costs upfront, but the final amount is confirmed after your insurance processes the claim.
We will send you an email invoice after insurance claim processing — please pay on time. We provide services in good faith that balances will be paid. If you overpay, we keep the amount as a credit for future visits. Contact us if you prefer a refund.
Medicare vs. Medicare Advantage
If you have enrolled in a Medicare Advantage plan, it is extremely important that you provide us with that information at your visit. While you will always retain your traditional Medicare number, you must use your Medicare Advantage plan information when visiting our clinic.

We offer competitive self-pay rates for patients without insurance or those seeking straightforward care. Fees are based on complexity of the visit.
Please note: we cannot provide comprehensive services to self-pay patients due to limited labs and medication availability.
CGM Placement & Data Review — Billing Notice
We want patients to be fully informed before their visit. CGM-related services are billed as two separate charges, each with its own insurance code, in addition to your office visit:
Covers in-office sensor placement, device setup and calibration, patient training on use and alarms, and printout of the recording. Billed at the visit when the CGM is placed or a new patient is trained on their device.
Covers provider download, analysis, and written interpretation of your CGM glucose data. Billed at follow-up visits when your CGM report is reviewed as part of your care.
